REQUIRED: Client Intake - please fax to 1.888.891.4673 no later than Noon on the Friday prior to your intensive.
OPTIONAL: Authorization to Release Confidential Client Information - please complete a copy of this form and fax it to 1.888.891.4673 whenever you want ChristianCounsel to forward your records to another mental health professional.
